Game Reschedule Form

Instructions for Game Reschedules:

  • Teams must agree to the reschedule date, time, and arena with signature or written confirmation before a reschedule request is submitted to District 2.   
  • Complete and submit the form below - be sure to include all required information.
  • Upon submission of the form, you will receive an email that summarizes the requested change. 
  • Print the email and mail it along with the $100 rescheduling fee (required for all levels) to Mark Mallory at the address shown below.  Make checks payable to "District 2 Hockey".
  • Email or snail mail a copy of the requested change to the opposing teams manager or coach.
  • The reschedule is not complete until you receive confirmation from D2. 
  • Contact Terry Thorsheim (contact info below) if you have questions about a game re-schedule.

In order to complete the online game reschedule form, you will be asked to login with a website account.  You can use your association's website account if you are on the NGIN System.  If not, you will be need to create a new account.
