Game Reschedule Form

Instructions for Game Reschedules:

  • Teams must agree to the reschedule date, time, and arena before a reschedule request is submitted to District 2.  Each reschedule is assessed a $100 change fee (make sure everyone is in agreement to the change)
  • Complete the entire form for the reschedule below - be sure to include all required information.
  • The Mandatory online payment for the 2017-18 season is set up again.  No request is sent for a game change until a Credit Card Payment is submitted..
  • You will receive an email that summarizes the requested change.
  • Email or snail mail a copy of the requested change to the opposing teams manager or coach.
  • THE RESCHEDULE IS NOT OFFICIAL UNTIL YOU RECEIVE A CONFIRMATION FROM A DISTRICT 2 OFFICIAL.
