• Once the form is submitted, you will receive an email confirmation from District 2.
• The confirmation will contain information on the fees that must be paid.
• Print a copy of the confirmation email and mail it along with a check made payable to "District 2 Hockey Officials Association" or "D2HOA" within 7 days to Neil O'Donnell at the address noted below.
